I hereby consent to let my child participate in La Salle Student Ministry hosted by La Salle Community Fellowship, including overnight retreats and related activities and travel.

I consent that photographs or video footage may be publicly taken of the minor by ministry leaders for ministry purposes only (which may include posting to social media and/or the ministry website). Media consent may be withdrawn by me at any time upon written notice.

I/we, the Parents or guardians named below, authorize one of the church’s Youth Program Personnel to provide consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment, or procedures for the participant named above, should it be necessary.

I/we, named below, undertake and agree to indemnify and hold harmless Program Personnel, the church, and its leaders from and against any loss, damage, or injury suffered by the participant as a result of being part of the activities of the church, including overnight retreats, related activities, and travel to and from church-sponsored events, as well as any medical treatment authorized by the supervising individuals representing the organization.

This consent and authorization is effective while participating in or traveling to or from events sponsored by the church, including overnight retreats and their associated activities. By signing below, I/we acknowledge that I/we have read and understood the information provided regarding the retreat and consent to my/our child’s participation as outlined above.
