You are receiving this invite because you have either been a part of the Youth program in the past, or you are entering Grade 6 and have been on the LSCF mailing list for VBS or other children's ministries.

Thanks for your interest in having your child participate in YOUTH! It's important for us to have up-to-date information so we can confidently care for your son or daughter at each youth event.

Even if you have filled out this form in the past, we ask that you resubmit all information including any information that may have changed for the current year.

Thank you for trusting us with your child, we don't take this opportunity lightly and will try our best to ensure your son or daughter has a phenomenal experience at each event.

Our Youth program will kick off on September 23, 2026, and run through early June 2027, with a break over Christmas.


Youth will meet weekly on Wednesday evenings at LSCF from 7:00–8:30 PM for Grades 6–8. Students in Grades 9–12 are welcome to hang out until 9:00 PM.


Consent for Youth Activities


To reduce the number of consent forms parents need to complete throughout the year, this registration form will also serve as consent for most Youth activities and outings that are considered lower risk. This may include activities such as going bowling, visiting a park, or travelling into Winnipeg as part of a youth event.


For activities that involve a higher level of risk, we will provide a separate consent form with specific information about the activity and any additional details parents need to know. Examples may include trampoline parks, sledding, or other activities where additional safety considerations apply.


Parents will always be informed about upcoming activities through the youth email. The email will indicate whether the activity is covered by the consent provided through this registration form or whether a separate consent form will need to be completed.

I hereby consent to let my child participate in the LSCF Youth Program hosted by La Salle Community Fellowship. This consent includes participation in regular, lower-risk youth activities, outings, and events, including activities held off-site and travel to and from church-sponsored youth activities in vehicles driven by church staff or approved volunteers. This may include transportation to Winnipeg or other nearby communities, as well as activities where youth walk or bike around the streets of La Salle.


I consent that photographs or video footage may be publicly taken of the minor by ministry leaders, for ministry purposes only (which may include posting to social media and/or the ministry website). Media consent may be withdrawn by me at any time, upon written notice.


I/we, the Parents or guardians named below, authorize one of the church’s Youth Program Personnel to sign a consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant named above.


I/we, named below, undertake and agree to indemnify and hold harmless Program Personnel, the church, and its leaders, from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of the church, as well as any medical treatment authorized by the supervising individuals representing the organization.


This consent and authorization is effective for the 2026–2027 Youth Program year when participating in or traveling to Youth Program activities sponsored by the church. Parents/guardians will be notified about upcoming activities through the youth program email and will be informed if a separate consent form is required.

Purposes and Extent: The church is collecting and retaining this personal information for the purpose of enrolling your Child in our programs, to assign the student to the appropriate classes, to develop and nurture ongoing relationships with you and your Child, and to inform you of program updates and upcoming opportunities at our organization. This information will be maintained indefinitely as it is a requirement of our insurance company and legal counsel. If you wish the church to limit the information collected, or to view your Child’s information, please contact the church office.
 
By typing your full name above and clicking submit you are electronically giving consent for your child to participate in this program. I have read, understood and agree with above and sign it to cover all Youth Program activities for the program year effective as stated below. A separate Informed Letter of Consent will be sent home for activities of elevated risk.
